This page explains how your information is used when you take the Leading Unmasked Mask Assessment.
Who is responsible?

What we collect and why
We collect your name and email address when you start, your responses to 21 statements, the seven calculated scores, the assessment and notice versions, your choice about a one-off email invitation, and the time you started and completed. Hosting and security systems may record IP address, browser information, and request data in technical logs. We use these details to run the assessment, show and email your summary, prepare a full report for coaching review, match a later discovery call booking, protect the service, and respond to privacy requests. We do not ask for health or other sensitive free-text details.
The assessment is a developmental reflection tool. It does not make decisions with legal or similarly significant effects, and it does not establish a clinical diagnosis.
Our basis for using the information
We use your details to fulfil the assessment and report request you make, and for our legitimate interests in providing the service and protecting it from misuse. The optional one-off invitation in your result email is sent only if you actively select that choice. Taking the assessment does not subscribe you to a newsletter or ongoing marketing.
Who receives it?
Hostinger provides the site and database hosting. Resend delivers assessment emails. Your full report is sent to report@maskassessment.com for Gavin’s coaching review; it is not automatically emailed to you. If you choose to book a call, you leave this site for Zoho Bookings, whose own privacy information applies. Service providers are given access only as needed for the agreed purpose. We do not sell your information.
How long is it kept?
An assessment started but not completed is kept for 30 days. A completed assessment, its report information, and associated delivery records are kept for 12 months after completion, unless an ongoing coaching relationship or a legal need warrants a different period. Technical logs are managed separately by the hosting provider. You can request deletion sooner using the contact address above, subject to applicable exceptions.
Security and transfers
Assessment details are transmitted over HTTPS. Contact details and responses are encrypted within the application before database storage. Access is limited to authorised people. Providers may process information outside the UK. Hostinger and Resend describe their transfer arrangements in their data processing terms and data processing addendum, respectively.
Your rights
Depending on the circumstances and applicable law, you may ask to access, correct, erase, restrict, or receive your personal information, object to certain processing, or withdraw the optional invitation choice. Email info@forumleadership.co.uk and provide the email address used for the assessment so we can locate your record. You may also raise a concern with a data protection supervisory authority.
Cookies and similar technology
This assessment does not use optional analytics or advertising cookies. The active questionnaire answers are kept in the open page’s memory until you submit or leave the page; refreshing the page starts again. Essential hosting and security technologies may operate to deliver and protect the site. Links to Gavin Bryce, The Forum, and Zoho Bookings do not load those sites merely by appearing on this page. Their policies apply when you visit them.
